6. Act 168 Employment History Form  (for current employer; also for any previous location where you have worked having direct contact with children)
    
7. Pennsylvania Mandated Reporter Training (online)
    
8. Staff Health Assessment
   
   
9. Diocesan Mandated Reporter Compliance Form
    
10. Diocesan Intent for Compliance (after reading the Policy referenced above)
     
11. Diocesan Creating a Safe Environment Inservice (online)​​​​​​
     
12. Recommendation Form
    --If Catholic - Pastor Recommendation Form
    --If non-Catholic - Character Recommendation Form
